Waiver nursing, supported and shared living, adult day support, residential respite and non-medical transportation - programs that extend care beyond the standard home visit.
Not every need fits a standard home care visit. Some people are funded through Ohio home and community-based waiver programs. Some need support delivered in a shared or supported living setting rather than in their own house. Some simply need a reliable way of getting to a medical appointment.
These programs sit alongside our core services and are arranged case by case, because eligibility, funding and the practical arrangements differ for each one. Several of them involve a county board, a case manager or a waiver program as well as us.
If you are not sure which program applies, or whether you qualify, the quickest route is to call and describe the situation. We will tell you what we can offer, and where you may need to go elsewhere.

Skilled nursing delivered in the home for individuals funded through a home and community-based waiver. This covers the tasks that fall outside what a caregiver is able to provide.
Ongoing support for adults living in their own home or apartment, built around the level of help each person needs to stay independent day to day.
Care and support provided in a shared household setting rather than in a person's own separate home.
Short-term care delivered in a residential setting, as an alternative to respite care provided in the client's own home.
Structured daytime activity and support away from the home, providing routine, supervision and social contact through the day.
Transport to appointments, shopping and community activities where no medical assistance is required during the journey.
That depends on the funding route and, for waiver programs, on an eligibility determination made by your county board or case manager rather than by us. Call and describe the situation and we will tell you which of our programs could apply and who you need to speak to about eligibility.
Home and community-based waiver programs fund care and support that allows a person to live in the community rather than in an institutional setting. Eligibility and funding are determined through the relevant program, not by Lifeway Cares - we deliver the service once that is in place.
No. Non-medical transportation covers journeys where no medical assistance is needed on the way - appointments, shopping, community activities. It is not an ambulance service and is not a substitute for medical transport.
Yes. Several clients use an additional program alongside personal care or homemaker services. Tell us everything that is needed and we will set out how the pieces fit together.
Call (419) 921-2144 or send a care request describing the situation, including any waiver or case manager already involved. That is usually enough for us to point you in the right direction on the first call.
